Before Your Initial Evaluation
- Make a list of questions you want to discuss with our Doctors before you arrive
- Create a list of signs and symptoms you’ve been experiencing in the last week, month and year
- In written format, present your medical history, previous treatments, current medications, and any other pertinent information that will help Alpha Therapy’s Medical team make informed decisions
- Bring in any X Rays or MRI’s you may have
- Call our front desk to get important insurance information
- We have great, convenient parking available but please show up 20-30 minutes BEFORE your scheduled appointment time to fill out paperwork
- Ensure that you bring a copy of your photo ID (driver’s license, military card or passport) as well as a copy of your insurance
- If you require additional assistance, make sure a friend or family is able to assist you to and from your appointment
What to Expect on Your First Visit
- Be greeted by our friendly staff and register at the front desk. They will assist you with any insurance processes and other intake paperwork
- Meet with Alpha Therapy’s Medical team for your initial evaluation
- Discuss your options and move forward with whatever works best for you
Please, help us help you! We need an open line of communication and total honesty so that we are in the best position to get you feeling better as quickly and efficiently as possible.
If you are taking medications, it is advisable to bring along a trusted friend or family who is familiar with your medical condition and may be able to shed light on what is in your best interest.
Once you’ve been assessed by our Physical Therapist, you will know your options and will exercise your freedom in deciding what you want to do, no pressure.
