If you hold a Commercial Driver’s License (CDL) or operate a commercial motor vehicle, a DOT physical exam is a federal requirement — not optional. Understanding what happens during the exam can help you walk in prepared and walk out certified. Here’s everything you need to know about getting your DOT physical in Silver Spring, Maryland.
What Is a DOT Physical Exam?
A DOT physical exam is a medical examination required by the Federal Motor Carrier Safety Administration (FMCSA) for commercial drivers. The exam confirms that you are physically and mentally fit to safely operate a commercial motor vehicle. Results are reported directly to the FMCSA National Registry and are valid for up to 24 months, or less if your examiner identifies a condition that requires more frequent monitoring.
Who Needs a DOT Physical?
You are required to maintain a valid DOT medical certificate if you:
- Operate a vehicle with a gross vehicle weight rating (GVWR) over 10,001 lbs
- Transport 9 or more passengers for compensation, or 16 or more without compensation
- Transport hazardous materials requiring a placard
- Drive a commercial motor vehicle in interstate commerce
Most CDL holders and long-haul truck drivers fall into one or more of these categories.

- Your CDL or commercial driver’s license number
- A list of current medications (prescription and over-the-counter)
- Glasses or contact lenses if you use them for vision correction
- Hearing aids if applicable
- Any relevant medical records (especially for heart conditions, diabetes, sleep apnea, or seizure history)
- Payment: $105 (cash, credit, or debit accepted)
What Happens During the Exam?
The DOT physical is thorough but straightforward. Here is what our certified medical examiner will review:
Vision Test
You must have at least 20/40 acuity in each eye (with or without corrective lenses) and a field of vision of at least 70 degrees. Color vision is also tested to ensure you can distinguish traffic signals.
Hearing Test
You must be able to perceive a forced whispered voice in the better ear from at least 5 feet away, with or without a hearing aid.
Blood Pressure and Pulse
Your blood pressure and pulse will be measured. If your blood pressure is elevated, the examiner may issue a certificate for a shorter certification period to allow for monitoring and treatment.
Urinalysis
A urine sample is collected to screen for underlying medical conditions such as diabetes or kidney disease. This is not a drug test.
Physical Examination
The examiner will conduct a head-to-toe physical including evaluation of your cardiovascular system, respiratory function, neurological condition, musculoskeletal system, and abdomen. They will also review your medical history for any conditions that may affect your ability to drive safely.
Common Reasons Drivers Are Not Certified
The following conditions may result in a shorter certification period or temporary disqualification:
- Uncontrolled high blood pressure (Stage 3 hypertension)
- Insulin-dependent diabetes (exemptions available through FMCSA)
- Untreated sleep apnea
- Seizure disorders
- Recent heart attacks or cardiac surgery
- Vision below the minimum threshold
In many cases, a condition does not automatically disqualify you. Our examiners will explain your options and any next steps required to obtain or maintain certification.
How Long Does a DOT Physical Take?
Most DOT physicals at Alpha Therapy take 30 to 45 minutes. Because we offer walk-in service on Fridays with no appointment required, you can plan your visit accordingly.
How Long Is a DOT Medical Certificate Valid?
A standard DOT medical certificate is valid for 24 months. However, if the examiner identifies a condition requiring monitoring — such as controlled high blood pressure or treated sleep apnea — they may issue a certificate for a shorter period (3, 6, or 12 months).
